· Brief Chronology with dates and locations pertinent to the case

· Detailed Chronology with pertinent information about the case, including date, Bates or other page numbers, extracted medical data, provider if available, and comments how the data relates to the case.

· List of records reviewed

· Any tables or charts necessary to explain the events
